1. Log in to your portal account
2. Click on the “Bills” Icon at the top
3. Choose your payment option
4. Click “Make Payment
5. Fill out the Card and Customer Information
6. Select “Make Payment”

Financial Responsibility
You are financially responsible for the cost of the mental health services provided to you or your dependent(s), including but not limited to deductibles, copayments, coinsurance, non-covered services, late cancellations, and missed appointments.

Forms of Payment
We accept cash, checks, debit cards, and most major credit cards.

Credit Card on File
We require a valid credit card to be on file.    MBC Credit Card Form

Insurance-Based Clients- Billing & Payment Process
You are responsible for any co-payment, co-insurance, or deductible at each visit in accordance with your insurance policy.

1. We submit patient claims to insurance.
2. When we receive the explanation of benefits (EOB) showing the patient’s responsibility (co-payments, deductibles, etc) if the patient has a balance, a statement is sent to the patient portal, and the patient is notified via text/email.
3. We charge the credit card we have on file for the balance due.

Insurance Updates
If there are any changes to your insurance, please provide us with your updated insurance information:  Insurance Update Form.     If your insurance becomes inactive, please note that you will be considered self-pay and be responsible for paying our self-pay rates before any future appointments.

Mind Body Co-op has a 24-hour cancellation policy for all individual sessions. Please call us at least 24 hours in advance to cancel or reschedule your appointment. Otherwise, you are subject to a cancellation fee. These fees are not billable to your insurance company and will be charged to your credit card on file.

*Group Psychotherapy requires weekly attendance and are charged a $30.00 missed session fee even if you do not attend.

To cancel or reschedule, please contact your provider directly and/or our Client Relations team at info@mindbodycoopchicago.com. Messages may be left by voicemail, as well, which will accurately record the date and time you called.

*Our Group Financial Policy states “I understand that all group therapy sessions at MBC require a commitment of weekly attendance. I understand that my insurance can not be billed if I do not attend the session. As a result, you will be charged a $30 fee for any groups that you do not attend, with or without notice.”

Getting to our offices, could not be easier. We have 2 Chicagoland locations and offer virtual services.

In case of an emergency or after-hours mental health crisis, please call 911 or go to your nearest emergency room. If you need immediate support, you can also contact the National Suicide Prevention Lifeline at 988 or the Crisis Text Line by texting HOME to 741741. It’s important to reach out to emergency services or crisis hotlines for immediate assistance in such situations.

Accepted Insurance Plans
Mind Body Co-op currently accepts most plans (Blue Cross Blue Shield PPO, BCBS Choice, Aetna, Cigna, Humana, and Optum), however, at this time, we do not accept HMO policies.

Out-of-Network: We are willing to submit billing to your insurance carrier for out-of-network benefits.

Depending on your current health insurance provider or employee benefit plan, it is possible for psychotherapy services to be covered in full or in part. Please contact your insurance provider to verify how your plan compensates you for psychotherapy services.

Please understand that although we may be in-network with your plan if you have not met your copay, deductible, and/or out-of-pocket expenses, Mind Body Co-op will bill you for those services after your insurance carrier processes your claim.

We recommend asking these questions to your insurance provider to help determine your benefits:

  • Does my health insurance plan include mental health benefits?
  • Do I have a deductible? If so, what is it, and have I met it yet?
  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
  • Do I need written approval from my primary care physician in order for services to be covered?

Records & Document Requests

To submit a request for our staff to complete a document, write a letter, or complete a request related to your medical records, please complete our Document Request Form.

Please allow 5 days to process this request.

Medical Records Release Authorization Form

For the release of medical records to be faxed or mailed, patients must fill it out and sign our medical records release authorization form. Please complete this form and attach the document to the Document Request Form.

Fees:

Unfortunately, we can not bill insurance for the completion of forms, so we have an additional fee to compensate the provider for their time to complete the form and we pass on any additional fees associated with the transmission of the data.

  • Request for Records or Treatment Plan:  $15.00 (uploaded to your patient portal)
  • Add-on fees: Fax: add $.05 per page,  Mail: add $.35 per page
  • Form Completion Request (FMLA, Accommodation Forms, Disability, etc.): $35.00 (uploaded to your patient portal)
  • + cost to Mail document, if applicable
  • Note: Must have attended 6 sessions with an LCPC/LCSW/Psy.D. or had 3 sessions with a Nurse Practitioner.  Your account must be in good standing before documents can be completed.

CareCredit

Mind Body Co-op is a CareCredit provider offering CareCredit bill pay as an additional payment option for your mental health services. Many patients put off treatment because they cannot afford to pay.  Care Credit billing allows you to pay over time instead of a lump sum prior to treatment.

Bone and Joint Specialists are CareCredit Providers offering CareCredit Bill Pay.

Patient Portal Prescription Refill 

You can request a refill for either a current or past prescription within the patient portal. Prescriptions and refills for patients of Mind Body Co-op are routinely issued during normal office hours.

Please allow 24-48 hours (excluding weekends) for your prescription refill request to be processed. Once you have submitted your request and allowed processing time, please check with your pharmacy that your prescription has been filled prior to contacting our office. We ask that you request your medications at least 3 to 4 days before your prescription is due to run out.

Self-Pay Clients

If you do not have insurance, payment is due in full at the time services are rendered. Our out-of-pocket rates are as follows and these amounts will be charged to your credit card at the time of service:

Nurse Practitioner (Medication Management)

  • Nurse Practitioners: Intake: $300 / Follow-Up: $200
  • Nurse Practitioner Interns: Intake: $200 / Follow-Up: $150

Therapy

  • Doctorate Level Clinician: Intake: $200 / Follow-Up: $150
  • Master’s Level Clinicians: Intake: $150 / Follow-Up: $130
  • Intern Clinicians: Intake: $125 / Follow-Up: $100

Family/Couples Therapy

  • Licensed Marriage and Family Therapist: All sessions: $200

Nutrition Counseling

  • Registered Dietitian: Intake: $150 / Follow-Up: $130

Group Therapy

  • Intake: $130.00
  • $30 per session/ $40 for groups with medication management
