Accepted Insurance Plans
Mind Body Co-op currently accepts most PPO plans (Blue Cross Blue Shield, BCBS Choice, Aetna, Cigna, Humana, and Optum), however, at this time, we do not accept HMO policies.
Out-of-Network: We are willing to submit billing to your insurance carrier for out-of-network benefits.
Insurance Verification
Please fill out our Insurance verification form and our billing team will verify if your insurance covers services at Mind Body Co-op. Our billing team will verify your insurance and send you confirmation of your benefits. The team at Mind Body Co-op (MBC) does our best to verify your insurance and inform you of your benefits at the onset of our relationship. However, if you have any questions about your benefits, it is in your best interest to contact your insurance carrier directly to understand your coverage.
Depending on your current health insurance provider or employee benefit plan, it is possible for psychotherapy services to be covered in full or in part. Please contact your insurance provider to verify how your plan compensates you for psychotherapy services.
Please understand that although we may be in-network with your plan if you have not met your copay, deductible, and/or out-of-pocket expenses, Mind Body Co-op will bill you for those services after your insurance carrier processes your claim.
We recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it, and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
Insurance Updates
If there are any changes to your insurance, please provide us with your updated insurance information by completing our insurance update form.
If your insurance becomes inactive, please note that you will be considered self-pay and be responsible for paying our self-pay rates prior to any future appointments.
