Your portal link will be available 24 Hours before your appointment.
You can access the link by going to your patient portal account. You will also receive an email to the address that we have on file.

To Access Your Patient Portal:
1. Go to your patient portal account. If this is your first time accessing please go HERE.
2. If you receive a “If your details match our records, we’ll email you a confirmation. No email? Contact us. If you already have an account, please sign in” message, and have not received an email from no-reply@advancedmd.com, please reach out to us at info@mindbodycoopchcago.com or call at +1 872-274-4344
Why was my appointment cancelled:
We require that your paperwork is filled out 24 hours prior to the appointment. Unfortunately, this deadline is not flexible. We do allow for a new appointment to be booked, and we can typically get you back on the schedule within a week.
If you think that your appointment was cancelled in error, please give us a call at +1 872-274-4344. You can also text us at this number if that sounds easier.
Getting to our offices, could not be easier. We have 2 Chicagoland locations and offer virtual services.
In case of an emergency or after-hours mental health crisis, please call 911 or go to your nearest emergency room. If you need immediate support, you can also contact the National Suicide Prevention Lifeline at 988 or the Crisis Text Line by texting HOME to 741741. It’s important to reach out to emergency services or crisis hotlines for immediate assistance in such situations.
